ETC Final Rule Resource Center


On September 18, 2020, the Center for Medicare and Medicaid Innovation (CMMI) within the Centers for Medicare and Medicaid Services (CMS) published a final rule to implement the ESRD Treatment Choices Model (ETC Model) with the goals of increasing rates of home dialysis and kidney transplantation for beneficiaries with ESRD enrolled in Medicare Fee-for-Service. 

The mandatory payment model covering 30% of adult ESRD beneficiaries will begin January 1, 2021 and end June 30, 2027. The final rule represents a key component of the HHS broader strategy to Advance American Kidney Health and “revolutionize the way patients with chronic kidney disease and kidney failure are diagnosed, treated, and most importantly, live.”
